Wiki Bundling for ENT procedures

Hi. I'm new to coding for ENT and have 2 scenarios I have questions regarding reimbursement.

First: We billed 31253 LT, 31255 59 LT, 30930 RT, 30930 59 LT, 31267 LT, 31256 59 RT, & 61782. Anthem CO is bundling CPT's 31267, 31256, & 61782. Is this correct. I can't find any other documentation that these are bundled.

Second: We billed 31255 RT, 31255 59 LT, 30930 RT, 30930 59 LT, 31256 RT, & 31256 59 LT. Again, Anthem CO is bundling CPT's 31256.

Please let me know if this is correct.

31253 inlcudes 31255 on same side , 30930 is bilat in nature only bill once no mods
without an op note, cant help, but 31253 originally billed is a combo code for ethmoids and sphenoid with not tissue removal
