hi

a doctor performed an midshaft osteotomy at the metaphyseal/diaphyseal region of the left first metatarsal. I can't decide between cpt codes 28295 or 28296. From reading different articles the diaphyseal region is the shaft (body) of the bone and the metaphyseal region is near the base. she also writes that the apex of the osteotomy pointed distally. diagnosis is hallux valgus

I would use 28295 instead of 96.

They usually cut the metatarsal head in a "V" shape, and then slide the MT medially.

If performed distally (28296) the hardware is often fixed in between the tarsal bones and MT.

The "Apex" is just refers to the V cut is all. The fact that is pointed distally makes no difference in choosing the code.
