My doctor did a illiac to anterior tibial artery bypass using a cryopresserved saphenous vein (CryoLife). I cannot find code that goes this far. What do I do? Thanks!

Also, We have a doctor that doesn't do coronary interventions. If he did the diagniostics and another did the intervention, same day same group. Do I need and modifiers? Didnt know since this isnt major surgery. Thanks.
