My surgeons don't often do this so I'm having trouble finding a code for this and worry it's unlisted. Anyone have some ideas for me? I appreciate the help!

Bypass of subclavian vein to right internal jugular vein using 6-mm PTFE graft.

I'm use to coding bypass grafts of the arteries, not the veins, that's what has me confused.
