We have two scenarios:

1. The patient is being seen for ante-partum care but has a history of a previous C-section. Which Dx would you use?
O34.219 or Z98.891

2. The patient is being seen for wound check for a C-section from 4-6 weeks prior (post-partum care). What dx would you use?



We are having a difference of interpretation in our office so any outside thoughts on this would be helpful.
 
Is the antepartum care a routine visit? If so, I would code with the routine ob visit codes. You can add Z98.891 after the routine code, but it should not be the primary code.

The incision check is usually considered routine postpartum care, I would code Z39.2.
