Good Evening to all:

My opinion in the below procedure performed : It is inclusive in the surgery. We may not bill it with 72040. Please share me yours thoughts.

The current thought that I have is we can bill for the technical component (TC) for x-rays/c-arm/fluoro if the radiology CPT code does not bundle with the procedure codes being performed and its very important to check the CCI edits.


Order Exam: C-SPINE AP AND LATERAL
CLINICAL INFORMATION: ACDF - CERVICAL DISC DISPLACEMENT MID CERVICAL REGION

FINDINGS:
Three fluoroscopic spot views submitted. Study demonstrates
intraoperative evaluation of anterior spinal surgery at C5-C6
with an anterior plate secured by screws and there is a
radiopaque disc space device present at this level. Anatomic
alignment maintained. Detail limited by fluoroscopic
technique. Better detail could be obtained with appropriate
postoperative radiographs which are recommended.


IMPRESSION:
Fluoroscopy provided for surgery as above.

76000 & 76001 are the codes for fluoroscopy/fluoroscopic guidance and are bundles into most surgical procedures. 72040 is not the appropriate code.
 
Could you please suggest whether the below procedure is also inclusive in surgery.

CLINICAL INFORMATION:
FUSION - L3-S1 DEGENERATIVE SCOLIOSIS;

TECHNIQUE:
C-arm films are submitted from the operating room

FINDINGS:
The images demonstrate transpedicular screw and posterior bar
fixation at L3-L4, L4-L5 and L5-S1. Interbody fusion devices
are seen at the disc spaces. The vertebral bodies appear to
be normal in alignment. Implanted stimulator is also
visualized.

IMPRESSION:
Postsurgical changes.
