when coding for this procedure (pAVF) and it is attempted and not successful how do you then code it? the same with a modifier? 52?

I assume you're coding for a facility if you're using these codes, in which case 52 is correct if there was no anesthesia involved in the procedure. If there was anesthesia planned or administered, then you'd use modifier 73 if the procedure was terminated prior to anesthesia, or modifier 74 if terminated after anesthesia.
