Can someone explain how many grafts were done here:Coronary bypass grafting x3 with LIMA to the LAD and reverse saphenous vein graft to the distal RCA and reverse saphenous vein graft to the first diagonal.

no it is not the correct code. read carefully. You asked how many grafts, you have 3 however they are not all arterial grafts. Go to page 215 of the CPT book where it explains exactly how to code this. I will not give you the answer I think you need to read the book on this one and discover it. I will assist if you get confused but it is well explained for you in the book
 
No 33519 is an add on code and cannot be listed alone. Did you read the instructions?
You have a combined arterial - venous graft.
LIMA - Left internal Mammary Artery - so that is one arterial
reverse saphenous vein to the distal RCA - that is one venous graft
reverse saphenous vein to the first diagonal - there is your second venous graft
For a total of three grafts one arterial and two venous
so you code the arterial graft as 33533
plus you code the add on code for the 2 venous grafts as 33518

As I said the CPT book does an excellent job of instructing you on this. It states for venous and arterial grafts you will need two codes.
