CAD with other angina

We have a coder who insist that the doctor should qualify what kind of angina if the doctor states CAD in his dictation.

Typically it is I25.118 CAD with other forms of angina. She insists that the doctor types in stable/exertional/equivocal angina in his notes.

We are confused by this bec we attended several other seminars and the speaker said it is not necessary. And yet with her know it all attitude, she still insists.

May I get a definitive answer from our auditing experts?

125.119 CAD with Stable Angina

I25.110 CAD with Unstable Angina

I25.10 CAD without Angina

I tend to agree with the coder. Our Physicians are not always that specific but it helps us get to a better DX for the patient when they are.
 
It would be better if they did document the type of angina, because I25.110 (unstable angina) is a higher risk category than unspecified and other. If you've got any plans paying based on HCC, coding the type of angina will ensure that the practice gets the appropriate resource allotment for the year.
