Wiki Calcium/Mag w/Oxaliplatin Coding

Hi,
We are trying to understand why this encounter would be coded like it is:

1.Calcium IVPB 9:25-9:57a (separate bags, one line)
2.Magnesium IVPB 9:25 -9:57a (separate bags, one line)
3.Zofran & Decadron 10:02-10:22a (same bag, one line)
4.Oxaliplatin 10:35a-12:40p
5. Calcium IVPB 12:50-13:25 (separate bags, one line)
6.Magnesium IVPB 12:50-13:25p (separate bags, one line)
7. 5 FU IVP 13:30-13:40p
8. Avastin 13:47-14:17p

Here is how we coded:
1. 96367
2. 96367
3. 96368
4. 96413, 96415
5. & 6. 96366 <--- why use this code for the 2nd dose of Cal/Mag (we are only getting credit for one med)

That is how i would code. The ca/mg is too far apart to use a second hour and from what i understand, you can only code 96367 once for the same infusate mix.
 
Based on what you coded, there is no credit for the second Cal/Mag. Why not assign 96366and why do we count the first Cal/Mag as a sequential but not the post administration as a sequential ( or get credit for two substances like we did in the pre administration).
