I have Calicum being denied because of the code V49.81(post menopausal). Insurance is saying that it is a rountine code and will not pay for it. The patient has normal calicum and no other DX for the calicum. What is everyone else using to get Calicum paid? Any help would be awesome.

If insurance does not cover routine care, then the patient pays. We don't code to get paid, we code to report what happened in the encounter.
