Wiki Camp, school, working papers with new providers that hasn't seen them

Hi! So I have new peds providers that has not seen the patients and they have already their wellchild visits within the year with the previous Dr. and they need paperwork filled out, the Dr. wants to bring them in and see them before filling out and signing off on it. How would you bill this and with what dx?

If the form is for sports or camp participation, in our office we charge a $25.00 fee that we do not bill to insurance. The provider does a sports/camp physical (They go by the form requirements). We use CPT code 99429 with dx Z02.5 (encounter for exam for participation in sport). I have attached an example of the form that our state requires to be filled out for a student to participate in a school sport.
