In our ASC, Physician performed lumbar decompressive laminectomies with partial facetectomies and foraminotomies at L3/4 and L4/5 with no discectomy peformed - only bony decompression. He then performed the same procedure at L2/3 removing a large extruded disc hernation. The provided diagnoses were lumbar spinal stenosis, disc herniation and sciatica.

Should this procedure be coded as 63047 for L3/4, 63048 for L4/5 and 63030-59 for L2/3?

Any input would be greatly appreciated. If you have a source stating the two primary procedures 63047 and 63030 can be reported on the same date for different levels, that would be great.

Thank you.
