Wiki Can 64999 be billed for a CRNA?

I ran into this with Ohio workers' comp - they deny 64999 for a CRNA stating servicing provider is not eligible for reimbursement for that code. They tend to follow Medicare guidelines. Are there any guidelines out there that unlisted codes (or 64999 in particular) can't be paid for a CRNA?
 
I don't know about other payers, but Ohio BWC rejected a manual adjustment request to pay 64999, stating the CRNA provider type can't be paid for that code.
