How would you report the last 31minutes of Leucovorin administration in the following scenario:

Oxaliplatin 10:30 - 12:01
Leucovorin 10:30 - 12:32

In other words, are 96413, 96415, 96368 the only three codes that can be reported or can 96366 be added to reflect the last 31 minutes of Leucovorin?

It looks like the Leucovorin and the Oxaliplatin ran concurrently so you would bill the 96368 to show the concurrent and 96413 for the first hour and 96415 for the 31 min - 1 hour.
 
I also bill 96366 to report the additional hour of LCV (the first hour was accounted for with 96368)
 
The 96368 does not have any additional time tied to it, it is simply stating that one drug is being ran concurrently with another. The additional hour for the Leucovorin is being accounted for as 96415 along with the Oxaliplatin. Even though the Oxaliplatin was stopped short 31 minutes before the Leucovorin, you are still billing the additional hour 96415 because the Oxaliplatin ran for 91 minutes. To bill an additional hour it has to be 31 to 60 minutes. 96415 is being billed for both the Oxaliplatin and Leucovorin additional hour and the 96368 is the code simply stating they are running concurrently.
