There have been patients bringing in forms that they need to get filled out for the school system in order to drive a school bus...Its not like a sports physicial, but there are certain things that have to be done. We are really confused about what to charge.....I would love for someone to take a look at the form and see what they think...I will be more than happy to fax it or email it to you.....please contact me at miranda.herbaugh@fmhhs.com
