I am VERY new at coding, so I have a few questions. I am doing a few homework exercises and need some help.
(1) a lateral lobe parotidectomy with facial nerve preservation. I chose code 42415. In the operative report it says that the "right side of her face was prepped and draped....". Should I be adding the modifier RT?
(2) excision of right arm lipoma. The mass was 4.0 cm plus surrounding tissues were excised. I chose code 11406. Should the modifier RT to added? I thought I read somewhere that skin in not considered a bilateral site so maybe a modifier shouldn't be added????
(3) a left video-assisted thoracoscopy with a wedge resection biopsy (left lower lobe) and a bronchoscopy with the right upper lobe, transbronchial biopsy times two. I chose code 32608 for the thoracoscopy and 31628 for the bronchoscopy. Do I use RT and LT modifiers for these procedures?
