Wiki Can you help me find coding guidelines with regard to OMT and ICD10 coding?

I'm a new coder and learning to code for outpatient family practice clinic. One of the providers is a D.O. and occasionally will perform OMT (osteopathic manipulative treatment) on a patient. There is a small set of ICD10 codes that need to be coded for the CPT codes for this procedure to meet medical necessity. The doctor does not always include the diagnosis codes in the documentation, but does describe in detail the areas where the treatment is performed. Can the ICD10 codes be coded from the provider's description, or should they be queried for more information?
