I am billing 99213,99833 and an injection (procedure) , Do I use mod 25 on both 99213 and 99833?
 
Modifier 25 is only applicable on Evaluation and Management CPT's.
You would not append a 25 to a procedure.

No worries. I didn't want to assume which code you meant ;)

You will use modifier 25 with 99213 if it is a separately identifiable service on that day. I ran the codes you listed through Codify, assuming 96372 is your injection code. There is no relationship with 90833 and it's an add-on code so - in theory - no modifier is needed on that code. Actual payer rules may vary and I recommend you look for a reimbursement policy addressing bundled services to determine how the payer wants these reported, especially if you're dealing with a denial.
