How would I code Adenocarcinoma of the lung with known mets to the liver.:confused:
 
you go to the neoplasm table to start look up lung then over to primary then liver and over to secondary . verify the codes in the tabular index and sequence first the one currently being treated. if both are being treated in the same encounter then sequence the primary site first and the secondary site second. If the lung is documented as treatment completed and no evidence of disease then sequence the liver first as the secondary site and the primary second as a V10 for history of.
 
If Tx/Cancer is still active for both then 162.XX and 197.7 but if Tx of Primary is complete then 197.7, and V10.1X. Just my 2 cents
