Wiki Capitation service and note

My Doctor says ,he doesn't need to have a documentation for each time patient comes for dialysis as long as patient is capitated and there is a note for multiple days in that month that patient came. is it true or not .

Go to the CMS website and download both their PIM and billing manual for dialysis. Whatever it says is required is what is required if you're billing Medicare. Otherwise you'll want to confirm with each payer what their documentation requirements are for visits pre-authorized
