We have been told that we can not charge for capped leads. We can only charge if they are removed. Is this correct? Where is it written? Any guidance is appreciated. Our Doctor says he was told we could so I need clarification. Thanks

Z-Health Publishing's Diagnostic and Interventional Cardiovascular Coding book states that capping of a lead is not removing a lead. It must me totally removed.
