Diagnostic Arthroscopy, Capsular perforation left knee

Arthroscope placed in lateral portal, no articular damage. Patellofemoral joint normal. [CPT 29870]

Patient had partial tear undersurface of posterior medial meniscus, fairly small. No tear superiorly. Two perforations with a spinal needle were placed in are of tear to promote vascularization and healing of meniscal tear.

Not sure what procedure code to use for the capsular perforation medial meniscus.

Code 29870 is off the table since a restorative procedure was performed. The other bad news is that this is now unlisted. This procedure is performed where the blood supply is not good, so the surgeon makes holes deep enough to cause bleeding, then the body heals itself.

Any suggestions on what I would compare the unlisted procedure to.

Hi Orthocoderpgu,

I have a question, what if a biopsy is being done with another arthroscopic procedure? Can we bill 29870 because there was a biopsy taken since the code is for a diagnostic arthroscopy with or without a biopsy?
