Good Morning-

I am in the process of update our charge tickets for Cardiac caths/interventions...peripheral angios and interventions plus EP...as you can see..I have alot on my plate...anyone started the process and have any forms they are willing to share..if so please fax to (904) 483-3123 attn Teresa or email to "tgembjork@aol.com
