Hello, we are looking for guidance/input on Cardiac Rehabilitation codes 93797 & 93798.

1. Does the cardiac rehab referral order reflect an initial order for basic exercise that would allow the patient to exercise during the initial evaluation visit? This visit would include basic exercise and at least 31 minutes of cardiac monitoring.

2.During the first visit the ITP, Physician-prescribed exercise and other required components for a cardiac rehab program (see NCD) would be created based off of the initial first visit evaluation?

We interpret this to be clear in the requirements for a cardiac rehab program however we question how does CMS expect Cardiac Rehabilitation departments to meet this requirement if they first have to assess and evaluate the patient to develop the ITP and Physician-Prescribed exercise to meet the program component requirements to bill either CPT code 93797 or 93978. Our question, can we bill CPT code 93798 for that initial first day visit and if not is there a CPT code we can bill??
