Wiki Cardio Exam element question

I see this situation a lot, and I want to find out how/or if it is counted.

In the exam, the physician says "CARDIO: normal rate and regular rhythm". Does this count as an exam element?

I think it might count in 1995 guidelines, but what about 1997 guidelines?
