can you bill 92960 (with appropriate modifier) at the start of a PVI 93656, provided the patient is in fib upon arrival?

can you bill 93657 with 93656 for additional afib lines?

both of these coding situations have been deleted sporadically by the hospital side billing, even though both scenarios are accompanied by appropriate MD documentation
