When performed, is a carotid angiogram included with the endarterectomy - 35301?

ALSO,

a separate case...

carotid endarterectomy was not done due to not a lot of plaque, but the patch angioplasty and carotid angiogram were done. How would this case be coded?

Carotid Endart

The angiogram is included in the endart unless it's performed on the opposite side and no prior diagnostics have been performed.

I'm not sure about the other one without more info. Was a duplex done and they saw stenosis and the planned procedure was an endart but once they got in there, they didn't do it? It seems like only the angiogram can be coded but again I'm not sure.
