Hoping someone can help with this. Patient was one day post op for a carotid endarterectomy 35301. The following day patient had a stroke and was returned to the OR for neck exploration and then an open placement of an iCAST stent into the common carotid artery. This involved opening up the neck incision and then a retrograde puncture of the left internal carotid artery with a sheath placed at the carotid bifurcation. A cathter was then placed in the proximal common carotid artery and an angiogram was performed which showed calcific plaque. We then advanced a 260 cm Rosen into the common carotid and removed the glide catheter. We then advanced the 7 French Ansel with it's dialtor tip into the common carotid artery. The dilator tip was then removed and we placed an 8x59mm iCast stent into the Ansel sheath. We marked up the carotid bifurcation so we were able to visualize on fluoroscopy the carotid bifurcation. We then unsheathed the stent in the common caritod artery by backing up the Ansel sheath and then deployed it by balloon angioplasty with an 8mm balloon.

I think the code is 37217 but would appreciate a second opinion.
