Wiki Case Management

I work in an urban clinic with all specialties. We have a case manager that calls and makes the referalls to specialists and coordinates the plan of care with outside providers. We have been told that we can bill for this service but that the code T1016 is not the correct code for this service.

Does anyone have any advice on what code to use?
