Our ASC does a lot of cataract surgeries. The most common dx our doctor dictates is nuclear sclerosis cataract and senile cortical cataract. With ICD-9 I'd use 366.16 & 366.15. Now with ICD-10 his office schedules with ICD-10 H25.811 (for right eye) when he says they have nuclear sclerosis cataract and senile cortical cataract. H25.811 is for combined forms of age-related cataract. I was coding H25.11 & H25.011 instead. Is there a rule on this? Do you code them separately like I've been doing or if they have more than one is it always "combined forms" H25.8--.
