Category III 0483T-0484T TMVR

Does anyone have any experience billing these new cat III codes. 0483T and 0484T. My doctors have done a handful and i keep getting denials for them. At this point I have no idea on how to get these paid. Help?????

We are having trouble with them too. Some are getting paid and some are not. (Have only billed them with Medicare) Documentation does need to be sent. But it's a toss up at the moment if they will get paid.
