Wiki Cath code sets

I've been out of cardiology coding for two years and have forgotten the "sets" that go together. I used to have a cheat sheet that told me if the docotor said " LHC, cor vis,asc aortogram" for example. I would know how not to miss ancillary codes. Lost said cheat sheet. Does anyone have something for me? The ones that I am seeing now are like the one above, "PTCA to RCA, LHC, Cor Vis"
Also, just for kicks and giggles: Exercise Myoview stress test, abdomial aorta us, carotid us, 2d echo B/L LE arterial US.
At this point in my memory I would code that last one
93015-26,78451-26,J1250,76700-26-59,93351-26-59-51 and I just feel sure that it is not correct.
I've been given a stack of echo's stress tests and caths to code and I 'd appreciate the input of someone who is currently doing it so I don't make to many errors!
