Interventional radiologist performed atherectomy/angioplasty fem-pop territory (37225), but couldn't cross occlusion from contralateral CFA access, so accessed below-the knee popliteal to complete treatment. Is there any way we can code separately for this 2nd access using a 59 modifier? Or are all catheter placements bundled?
 
All code includes catheter placement. Selective catheterization for diagnostic angiography may only be reported if through a separate puchture site. Diagnostic angiographiy imaging codes maybe reported and modifier -59 is required if on same date/session.

You may look at mod -22 Increase Procedural Services and of course it has to be documented for medical necessity.
