If a CC states a particular test is going to be done but in the exam it is not documented by the provider (but by a student), the provider documented the test in the A&P though, can this still count as billable? The exam met all other documentation requirements...please advise. Thanks!
 
You need to clarify your question

I'm sorry but I do not understand your question.

Are you saying that the chief complaint is something like: Here for stress test?

Well, that wouldn't be documented in "the exam" in any case.

Then you say a student documented the test? But the physician documented the test in the Assessment and Plan?

Are you asking if the "test" is billable? Or are you asking if the E/M service is billable?

Please provide a more clear scenario and I'll try to help.
