Wiki CCM and BCBS

Is anyone seeing denials from BCBS for chronic care management where nothing else is billed on the same day but the denial is that the 99490 is "incidental to itself??" If so, how are you handling these? I am not really sure how something can be incidental to itself.

Has the charge already been billed during the calendar month ?

No. The only thing billed in a 99213 three days before the CCM. I think BCBS has come up with their own policy to get out of paying. We are starting to see this denial only with them and on other procedures as well.
