I have a few questions regarding billing for CCM
1. You have 2 chronic condition care plans
(Prostrate cancer and hypertension). Clinical staff documents for the monthly charge. Do you need to address the chronic conditions and use the diagnosis that is involved in the care plan? Sometimes there is no mention of this. The documentation will talk about transportation issues or problems getting a script filled for a different chronic disease but no plan is created. I have read the 15 page Medicare document but it doesn’t answer this question. Any help is appreciated
