Good Morning,

We are entertaining the idea of having the physicians document and care for the patients and employ coders to do what they do best(Novel concept huh?).

This seems simple, but I am trying to figure out how this would work with our EHR as the diagnosis are linked with the code. There are many times when a more speicific code could have been used as well as the software does not give alot of detail on the code and many time it can be incorrect. With ICD-10 implementation our providers are going to have to concentrate on improving documentation.

Any thoughts? Ideas? Thank you!!! :)

I have worked with 2 different EHR programs. Neither had good diagnosis descriptions and some were down right wrong. As a coder, you can either extract the information from the notes and apply the correct code or query the provider. At any rate, there should always be a coder involved before a claim is submitted.
