if patient had to go to operating room for retained cerclage that we initially placed am i able to charge for it because she had to go to the operating room and the difficulty or is it still not chargeable since we initially placed it.

The codes for cerclage removal require that a general anesthetic was used during the removal. If this was the case, you may bill for the removal. The CPT Assistant published the following: Subsequent removal of cerclage sutures at the time of delivery is considered part of the delivery and is not reported separately. However, removal of the cerclage suture under anesthesia before the onset of labor, or at a time that extends beyond the global period of the placement procedure, is reported separately with code 59871, Removal of cerclage suture under anesthesia (other than local).
 
my understanding is that if our providers placed the cerclage then we cannot charge for the removal. but just to be super clear i can charge for it even though we placed it because it extended beyond global or done under anesthesia
