I noticed they don't say to bill 17003 in units for each additional lesion. It now says "LIST SEPARATELY IN ADDITION TO CODE FOR FIRST LESION"

Does this mean I have to do this for say 8 lesions? But the claim forms only has room for 6 CPT codes so I'm confused. Am I not allowed to bill units for this anymore?

17000 is for the destruction of the 1st lesion
17003 the add-on code (1 unit ) would be for second up to the fifteenth lesion. I am not certain but I think that is the correct interpretation.
This would leave room for 4 more line items (HCPCS ) codes. You can always go to a second claim form if you run out of room.
