Wiki Charging for medications with no documention

Okay can someone tell me where to find the rule on this...can we charge for medications given orally, the doctor ordered they checked it off the medication list that it was given and the nurse initialled it next to the doctors order but did not document when, how and what time she gave the medication.
I know the answer but need to back it up
