Wiki Charing a UHC . . .Private Pay?

There is a man in our community (with United Health Care) who needs daily wound dressing changes. The wound is located behind his shoulder making it impossible for him to do the dressing change himself, his wife can't "stomach" it, and his PCP is 30 minutes away. We are trying to help him out, but we are aware that we can't bill for the dressing change accept by the E&M code. Since we are a hospital, I don't know of any E&M code that we can use. (It's not ER, not OBS, not IP.) We are contiplating putting the patient in private pay and letting him pay us a small fee if he agrees. However, we can't come up with a price based on CPT, so we would have to pull something out of the hat I suppose.

I'm concerned that this might not be the right thing to do. Can we charge him something that we can't charge his insurance? If the answer is no, what is our options? The only other option I see is sending him to his PCP where they can charge an office visit, but that's out of town.
