If you are a physician coder, then 78465 and other codes are done with 26 modifier telling that your provider read the stress.
Now if the EKG was read by another MD (eg. like a treadmill walk)then that physician would use 93016 and 93018 as respectively to the situation.
And the third scenario is when you own the whole equipment and did the entire scenario in your office then it is global biling as 78465,78478,78480, the drug codes and 93015.

To answer specifically to your question, stress test is part of the codes but it really has to be seperated depending on the three scenarios discussed as above. Hope this helps you.
