chemo for 3 hrs?

pt. had Etoposide chemo infusion from 11:05 to 3:05

would i code 96413 for the 1st hr.
and 96415x2?

11:05 to 3:05 is 4 hours total.

96413 for the 11:05 to 12:05 hour
the 2nd hour starts at the 91 minute threshold so, if the chemo stopped at 1:36pm you can code the additional hour CPT, eventhough you did not technically have a full 60 minute 2nd hour.

I would bill this scenario above as:
96413 x 1
96415 x 3
