Aetna seems to deny a chemo injection with an E/M. I coded an E/M with modifier 25 with the chemo injection code? Any suggestions? Thank you

chemo is almost always planned in advance therefore you cannot code an E&M for a patient to receive a preplanned injection/service. They are paying for the lesser of the 2 services most likely, which they are allowed to do.
