Coding for Rituxan infusion 96413,96415,J9312. Total of 145 minutes. WPS (IN) MC denying 96415 as bundled code. Is anyone else seeing this issue?

96415 is an add on code and should not be bundled as long as it is one hour after the initial hour and performed in the physician's office.
 
Are you billing as hospital outpatient? If that's the case, the EOB for the UB04 claim may show the drug paid but the admin codes may be bundled with the payment of the drug. Is 96413 getting paid?
