Hello all:

Would you consider "Hospital F/u" a CC?

Any thoughts?
 
I would not consider this a complete CC, unless there is more detail in the note to make it clear. Here's the official definition: "The CC is a concise statement describing the symptom, problem, condition, diagnosis, physician recommended return, or other factor that is the reason for the encounter." I always ask my providers to document specifically what problem(s) are being followed rather than just stating the visit is a follow up.
