Home Health Certification Denial

I have a claim for home health certification(G0180) being denied as content. The only other charges I have is an office visit the day before. Can anyone give me any information on this or direct me to an appropriate website with this information.

My first question would be, Did another provider bill for this as well? and are you billing Medicare or a Private Ins?

We are billing BCBS Kansas. As far as I know no other doctor is billing for this service.
