Our clinic is putting things into place to start Chronic Care Mgmt. We are an urban clinic where part of our population has trouble getting in to see us. I have several visits where the NP saw a patient for a visit specifically for their chronic issues, but then the doctor saw them also. Shouldn't these two encounters be combined for one E/M code? Others are being seen by the NP for their CCM but then the Dr. on another day. Would these all fall into the 99201-99215 range? I'm not seeing any source that says different.
