Can someone tell me for sure what makes a condition "chronic"? A medical dictionary will tell you it's any condition that lasts 3 months, but a webinar I recently watched said that when you see a patient a second time for the same condition it is considered chronic. That's a big difference. Does MCR have it's own definition?

A condition is chronic when the provider specifically tells me that it is. I make no assumptions.
